Transitional Living for Youth Who Have Completed Residential Substance Abuse Treatment
The BYF Substance Abuse Transitional Housing Program provides youth who have completed residential substance abuse treatment, additional treatment in a supportive less restrictive environment.
To be eligible to participate in the program the youth, ages 12-17, must have completed a residential substance abuse treatment program. Participants must also meet criteria for placement as established by the American Society of Addiction Medicine for Level III.I: Clinically Managed Low-Intensity Residential Treatment.
Services Offered by Transitional Housing Program
- Outpatient substance abuse treatment groups (provided through MK Place) - strong emphasis on relapse prevention and social support per The Matrix Model
- Case management.
- Preparation for independent living (bank account, job skills, getting birth certificate,etc)
- Family counseling.
- Mental health counseling.
- Healthy lifestyle practices including recreational opportunities and constructive leisure time activities.
- Social Skill Development.
- Transportation to school, work, and other appointments.
- Home living skills and responsibilities (keeping room clean, etc.)
- Medication management.
- Liaison between the school and the parent/guardian.
- Random drug testing.
- Behavior management (level system).
- Monthly progress reports sent to parent/guardian.
